Online application and required materials must be submitted by Feb. 28, 2022.

  • All materials must be emailed to: doa.youth.involvement@doa.nc.gov 
  • Applicants who fail to submit all required information will be ineligible
  • Please carefully check to ensure that your application packet is complete and includes all required documents.

  • Please use the form below to start the Lady Cardinal Mentorship Program application process. Once the form is submitted, you will received an email with a copy of your information plus next application process steps. 
  • Email the following to doa.youth.involvement@doa.nc.gov 
    • Submit cover letter
    • Submit resume
    • Submit High School Transcript(s). Unofficial copies are accesptable.
