Belmont Lake Driver & Belmont Villas Way: *1200 feet of 4" HDPE conduit 2. S WINSTEAD & BEACHWOOD : *2500 feet of 4" HDPE conduit 3. Lawrence Cir: *1000 feet of 4" HDPE conduit 4. Business Park Drive: *1800 feet of 4" HDPE conduit 5. 160 S WINSTEAD AVE: * 400 feet of 4" HDPE conduit 6. Cunningham Drive: * 1700 feet of 2" HDPE conduit * 450 feet of 3" HDPE conduit 7. Covenant Ct: * 2200 feet of 2" HDPE conduit * 200 feet of 3" HDPE conduit 8. 3031 Treatment Plant Rd (Wastewater Treatment Plant) – 800FT of 4” Conduit The City will supply all 2”, 3", and 4" HDPE conduits. The company awarded the contract shall be responsible for all labor, transportation, and insurance needed to perform the project in a safe and workmanlike manner for acceptance by the City of Rocky Mount. The Company awarded the contract will immediately remove trash or debris, generated by their work, from each job site and dispose of it properly per state and local regulations and policies. All work is to be performed in a safe manner in compliance with all accepted common industry practices and all OSHA, federal, state, and local regulations. Any damage(s) caused by the Company awarded the contract while performing the contract requirements shall be the sole responsibility of the Company and shall be remedied by the Company before payment is rendered by the City. The Company awarded the contract will be representing the City of Rocky Mount and shall always display a professional appearance, consisting of FR uniforms of at least 4 ATPV as well as all other NESC or OSHA required Personal Protective Equipment (PPE). PPE shall always be worn by the Company employees. All work performed inside the transformer shall be done by a qualified employee. Work will not be given to a sub-contractor without the expressed written consent of the City of Rocky Mount Electric division. The Company awarded the contract will be responsible for landscaping and restoration in all disturbed areas of the project to similar or better condition in accordance with the following: Apply topsoil and fertilizer, spreading topsoil w/ a rototiller or suitable equipment. After tilling the area, rake, and grade to final grade with gentle slopes for adequate drainage. The finished grade shall be even and uniform w/ sidewalk and curbs or borders. Seeding - seed should be of the same variety of the existing grass, shall include appropriate seed for the environment, and be applied using a seeder or broadcast spreader. Cultivate the area to a depth of six (6) inches removing weeds, stones, sticks, and clods larger than 2" in diameter. Topdressing to be applied immediately following seeding and will be either hydro-mulch or straw. If the straw is the selected top dressing, it should be partially embedded into the soil with a crimper or similar implementation tool. Where sod is present, it shall be carefully removed and stored. Sod shall be reinstalled on well-tamped soil after work is completed. The City reserves the right to adjust the estimated 6900 feet of 4" HDPE conduit, 650 feet of 3" HDPE conduit, and 3900 feet of 2" HDPE conduit of boring to be performed, should it become necessary. The company awarded the contract should be prepared to address all traffic control concerns, as required by the city representative. The City will not supply traffic control devices. The company awarded the contract is responsible for repair to the roadway, sidewalks, and curbs if it is damaged or cut for locating and or construction.
