North Carolina Department of the Secretary of State Advance Health Care Directive Registry
Three (3) Advance Health Care Directives forms and an Organ Donor Card form are located here for your information and use. The Organ Donor Card requires two witness signatures. You may complete the forms online and print the completed forms for notarization. If you prefer, you may open the blank forms with Adobe Acrobat ™, print the blank forms, and complete them later. The documents will be made available online when submitted to the Secretary of State's Office with a $10 fee per document. Learn More...
