North Carolina Department of the Secretary of State Advance Health Care Directive Registry

Three (3) Advance Health Care Directives forms and an Organ Donor Card form are located here for your information and use. The Organ Donor Card requires two witness signatures. You may complete the forms online and print the completed forms for notarization. If you prefer, you may open the blank forms with Adobe Acrobat ™, print the blank forms, and complete them later. The documents will be made available online when submitted to the Secretary of State's Office with a $10 fee per document. Learn More...

The Community Resource Information System (CRIS) helps local communities in North Carolina obtain information about state government financial assistance and other programs. Designed as an easily-used, customer-focused Web resource, CRIS enables local governments, small businesses, nonprofit organizations, state agencies, legislators, and citizens to locate quickly source for government grants and other types of assistance.

Begun in the late 1990s as a cooperative effort among the Office of State Budget and Management, the State Library of North Carolina, the Department of Commerce, and other state and local agencies, CRIS has evolved to take advantage of technological innovations.
